What to Expect: Your First SaS Pain Management Appointment

Plan to arrive 30 minutes prior to your scheduled appointment time and to spend approximately 90 minutes total at our office.

Bring the following documents with you:

• Photo ID
• Insurance card(s)
• Your completed New Patient Health History form
• A signed copy of the Informed Consent and Opioid Agreement
• Your completed HIPAA Privacy Policy, Late Policy Consent Form, Notice to Bill
• Any relevant medical records and diagnostic films (e.g., x-rays, MRI results)
• Your referral form from your primary care physician (if required)

Please also review the SaS Pain Management Privacy Policy and print a copy for your records.
